Outcome Tracker is committed to protecting the confidentiality and security of information we collect about individuals. This document describes the type of information collected and the purposes for which it is used. This policy is effective on May 16, 2007 and can be changed at any time at our sole discretion.

Information is Collected Automatically
When you visit this site, your current IP address, the type of operating system you are using, and the type of browser software you are using is recorded. This information is collected in aggregate and is used only to enhance your immediate user experience and inform future enhancements to the site. No personal identifying information is collected automatically.

Information Is Collected Voluntarily
In order to use this site as designed, you will be asked to provide personally identifiable information at registration. You will also be asked to provide answers to questions regarding your symptoms and treatment. If you do choose to provide us with personal information, you can be assured that such information is kept in secured databases.

How the Information is Used
There are two general purposes for which information gathered at this site is used:

1. Personally identifiable information is used only to inform and enhance the treatment provided by your doctor. This may include treatment recommendations, appointment reminders, and other medically relevant services performed by your doctor.

2. Certain statistical information is also gathered at this site by analyzing the information provided by all or certain groups of users in the aggregate. This statistical information is used for research purposes in order to evaluate the efficacy of treatment.

Disclosure to Third Parties
The only reason we will disclose what we know about you to third parties is if we are doing what a court or government agency requires us to do; for example, complying with a search warrant or subpoena.

Marketing
We will not share information about you with anyone for use in marketing products to you, unless we first notify you. You will then have an opportunity to tell us not to share your information by "opting out."

We will not share your health information with any other company, even one of our affiliates, to permit it to market its products and services to you.

Your Right to Access Information
Generally, you will have access at all times to your personally identifiable information as a part of your ongoing interaction with your doctor.
